Mit der Einf\u00fchrung einer neuen, weltweit einheitlichen Parodontitis-Klassifikation am 21. Juni 2018 wurde auch die Periimplantitis neu definiert (Berglundh et al. 2018, Schwarz et al. 2018). Demnach ist eine Periimplantitis gekennzeichnet durch Entz\u00fcndungen im periimplant\u00e4ren Bindegewebe und fortschreitenden Verlust von St\u00fctzknochen. Schon fr\u00fch w\u00e4hrend des Nachsorgeprozesses ist der Beginn einer Periimplantitis m\u00f6glich.<\/span><\/p>\n

Auf histopathologischer Ebene zeigt die Periimplantitis deutliche Unterschiede zur Parodontitis. Die Periimplantitis hat ein h\u00f6heres Progressionspotenzial und eine im Vergleich erh\u00f6hte lokale Entz\u00fcndungsantwort. Die Erkrankung schreitet nicht-linear und \u2013 verglichen mit einer Parodontitis \u2013 beschleunigt voran (Schwarz et al. 2018).<\/span><\/p>\n

Was ist gesund und was schon krank?<\/span>
<\/b><\/span>Periimplantitis-Stellen zeigen klinische Anzeichen von Entz\u00fcndungen und erh\u00f6hte Sondierungstiefen im Vergleich zu Ausgangsmessungen.<\/span><\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n

Jedoch sei es allein mit der Sondierung bzw. Messung der Taschentiefe nicht m\u00f6glich, gesundes von krankem periimplant\u00e4ren Gewebe zu unterscheiden (Berglundh et al. 2018).<\/span><\/p>\n

Die periimplant\u00e4re Mukositis entspricht der Gingivitis an nat\u00fcrlichen Z\u00e4hnen und gilt als reversibel. Darum ist eine fr\u00fchzeitige Diagnose und Behandlung wichtig. Bleibt eine periimplant\u00e4re Mukositis jedoch unbehandelt, kann die Entz\u00fcndung auf das umliegende Knochengewebe \u00fcbergreifen, es kommt zur irreversiblen Periimplantitis, welche zum Verlust des Implantats f\u00fchren kann.<\/span><\/p>\n

Wenn es also gelingt, Risikopatienten zuverl\u00e4ssig und fr\u00fchzeitig zu erfassen, besteht eine gro\u00dfe Chance, die Pr\u00e4valenz der Periimplantiden zu reduzieren.<\/span><\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n
